Definition

The rectum, an organ located at the lower end of the large intestine, is situated between the sigmoid colon and the anal canal. The anal canal lies between the rectum and anus. The condition involves an abnormal fibrotic narrowing or constriction characterized by a decrease in the size of the rectum or the anal canal or both. Rectum and anus, stricture of may be referred to as anorectal stenosis or contracture.

Etiology

Probable causes of the condition include: trauma; scarring after anorectal surgery or inflammation; radiation; and laxative abuse.

Signs & Symptoms

Manifestations of the condition include: dilated veins in the anorectal area; difficult defecation; painful defecation; leakage; retained fecal material (impaction or constipation); watery stools; abdominal cramping; pain; bleeding; and the inability to defecate.

Tests

Other than objective examination by visual assessment and digital examination, there is no specific test indicated for diagnosis of stricture of the rectum or anus. Significant testing may be necessary to determine the underlying cause or causes.

Treatment

Treatment may involve surgical removal of the scar tissue, or digital or instrumental dilation. A colostomy may be indicated. Drug therapy is indicated if the underlying cause is due to infections.

Residuals

If the person responds to treatment, no residuals are anticipated. If a colostomy has been performed, the person needs to wear a drainage bag for collection of drainage. Meticulous skin care around the area of attachment of the feces bag, and diet modification to avoid certain foods are needed. Mental depression may develop after a colostomy, and psychological counseling may be recommended.

Special Considerations

  • May be entitled to special monthly compensation where the Veteran has a single service-connected disability rated as 100% with additional service-connected disability or disabilities independently ratable at 60% or more, which are separate and distinct from the 100% service-connected disability and involves different anatomical segments or bodily systems. See  38 CFR 3.350(i)(1) – Special Monthly Compensation (SMC).
